استبيان اهتمامات الوالدين
يرجى من أولياء الأمور إكمال الاستبيان التالي إذا كنتم ترغبون في أن يتم إعلامكم والنظر في فرص المشاركة في لجنة أولياء الأمور في المستقبل. تشمل الأمثلة على فرص اللجنة، على سبيل المثال لا الحصر، المقابلات واجتماعات أصحاب المصلحة.
يرجى ملاحظة أنه سيتم إخطار جميع أولياء الأمور بالأحداث المدرسية بأكملها مثل التجمعات وأحداث ما بعد المدرسة والاحتفالات واجتماعات رابطة الأهالي والمعلمين . ليس المقصود من هذا النموذج الاهتمام بدعم هذه الأنواع من الأحداث المدرسية بأكملها.
